The identification card must be issued by the United States Government or a State or Tribal Government.

  1. Drivers License or State issued identification card

    • Requirements- Unexpired, contains the customer’s photograph, signature and physical description. Must include: Height, weight, eye color, hair color.
    • Issued by: A State or Territory of the United States.

  2. US Passport

    • Requirements- Unexpired.
    • Issued by – United States Department of State.

  3. Armed Forces ID card

    • Requirements – Unexpired.
    • Issued by – Any branch of the United States Armed Services.

  4. Other Government Issued ID cards

    • Requirements- Unexpired, contains the customer’s photograph, signature and physical description. Must include: Height, weight, eye color, hair color.
    • Issued by – United States Government, a state, US Territory or Tribal Government.


If you are unable to provide one of these ID cards. You may be identified on the oath or affirmation of a credible identifying witness who has their own approved ID card. Credible Identifying witnesses must have personal knowledge of your identity, believe it is not reasonable for you to obtain the approved ID, and be honest, aware and impartial.
